Registration for 2023-2024 will open Wednesday, August 2nd.

2023-2024 Registration Letter

Registration for the upcoming school year will be required to be completed online through E-Registration on PowerSchool. Student registration fees and lunch fees can be paid online through RevTrak. If you are unable to complete either process online, please call Sarah or Brandy at the high school ((563) 922-2091) to set up a time to come in. This will be by appointment only.

Please make sure all contact information (cell phones/email/emergency contacts/etc.) are updated. Please don’t list parents as emergency contacts. Text message alerts will be coming from the high school phone number ((563) 922-2091) and emails will come from sender mvalerts@maquoketa-v.k12.ia.us. If the information in PowerSchool is not accurate,
you will not receive these alerts. Contact Sarah Lown if you have questions.

Maquoketa Valley is partnering with the Northeast Iowa Food Bank to provide the BackPack Program to students during the 2022-2023 school year. The program begins in September and will continue throughout the school year. For more information and to register, please see the BackPack Program letter.

Free/Reduced Applications

Returning student registration process and/or adding a student to your account.

To E-Register, please log onto your PowerSchool portal by using the following steps:

  1. Go to www.maquoketa-v.k12.ia.us
  2. Click on PowerSchool link on the right-hand side.
  3. Enter your username and password.
  4. Once you log in, your child’s name should appear in the upper right. Go to the bottom left-hand side and select FORMS. (If you are adding a student, click on ACCOUNT PREFERENCES and STUDENTS then ADD.)
  5. Complete all of the tabs (demographics, language, health, contacts, contacts sort, permissions, fees and finish) then click on save, then submit when finished (this process should be done for each child in the district).
  6. On the left-hand side, click on UPDATE CONTACTS. Make sure each person listed has a relationship selected for their name.
  7. On the left-hand side, click on SORT CONTACTS. Make sure they are in your preferred order, then save.

**Now that you have registered your child(ren), you need to pay for registration and lunch fees**

  1. Click on the REVTRAK link on the left-hand side.
  2. Enter your username and password for REVTRAK, which is separate from PowerSchool.
  3. Click on POWERSCHOOL FEES for registration fees and FOOD SERVICE for lunch fees.
  4. You will be directed to enter in your debit or credit card information at checkout.
  5. Click on COMPLETE ORDER and you may view and print your receipt if desired.

**Fees for registration or lunch can always be dropped off at the school or sent with your child**

**If you forgot your RevTrak password, please click on forgot password to reset**

Students new to the district.

If you are new to the district, you will need to create a PowerSchool account by following these steps:

  1. Go to www.maquoketa-v.k12.ia.us
  2. Select POWERSCHOOL link on right hand side.
  3. Select CREATE ACCOUNT and again CREATE ACCOUNT.
  4. Fill in your parent information, create your own username and password (which must be 6 characters long) and fill in your child’s name, access ID, access password and relationship.
  5. You will then be directed to the PowerSchool login page to enter the username and password you created.
  6. Once you login, go to the bottom left-hand side and click on FORMS.
  7. Complete all of the tabs (demographics, language, health, contacts, contacts sort, permissions, fees and finish) then click on save, then submit when finished.
  8. On the left-hand side, click on UPDATE CONTACTS. Make sure each person listed has a relationship selected for their name.
  9. On the left-hand side, click on SORT CONTACTS. Make sure they are in your preferred order. Then save.
    **Now that you have registered your child(ren), you need to pay for registration and lunch fees**

Student punch passes are available for purchase in the High School office for $30.00. Students get one card with (10) punches to be used at any High School or Middle School activity. It can be shared amongst siblings if you choose to do so. Any checks made out to purchase a punch pass should be made out to MV Activities.

The following health forms needed before the new school year begins. Be sure to schedule any physicals early. The health form will need to be completed for each child attending Maquoketa Valley Schools for the 2022-2023 school year.

  • Health Form

  • Dental Screening Certificate: required for all 8th graders entering 9th grade
  • Dental Screening Exemption Form

Preschool

Junior Kindergarten/Kindergarten

  • Physical form completed by doctor’s office.
  • Updated immunization record (or medical/religious exemption form).
  • Vision form - may be completed by an eye doctor, medical doctor, nurse practitioner, or physician assistant. Screenings are considered valid if done no earlier than 1 year before and no later than 6 months after school starts.
  • Dental form - screening may be performed by a licensed dentist, dental hygienist, nurse, nurse practitioner, or physician assistant. Screening is considered valid from age 3 years to 4 months after the start of school.
  • If your student attended Junior Kindergarten during the previous school year and turned in the required forms, no additional forms are needed.

3rd Grade

  • Vision form - may be completed by an eye doctor, medical doctor, nurse practitioner, or physician assistant. Screenings are considered valid if done no earlier than 1 year before and no later than 6 months after school starts.

7th Grade

9th Grade

  • Dental form - screening MUST be performed by a licensed dentist or licensed dental hygienist. Screening is considered valid if done no earlier than 1 year before and no later than 4 months after school starts.

12th Grade

  • Updated immunization record (or medical/religious exemption form) with proof of having had the Meningococcal (meningitis) vaccine. 2 doses of meningitis vaccine are required for students, if born after September 15, 1999; or 1 dose if received when student was 16 years of age or older.

All medication must be in the prescription container that has been filled by a pharmacist. It maybe necessary to obtain an extra container from the pharmacist, since only the amount needed is to be sent to school. The parent or guardian must sign a permission form to have the prescribed medication dispensed to their child according to the specific written directions of the prescribing health provider. All medication will be dispensed by the nurse or authorized office personnel who have met the qualifications by taking the medication administration course as stated in chapter 204 of the Code of Iowa.

All medication will be maintained in a secure location and appropriately identified for each child.

All medication permission slips will be filed in the student's health file in the nurse's office.

Any medication that has not been taken or picked up at the end of the year will be destroyed.

It is strongly encouraged that parents bring their children's medicine to school and not have the children carry it.
